How to Participate

Please click on the participate button below to access the online questionnaire. If you are logged in from a mobile device, you will only be able to access a basic questionnaire with multiple choice questions. For accessing the long questionnaire, please log in from a desktop or a laptop.

Once you have filled out and submitted your information, it will be live on the map. You can edit your information at any time.

Before you use this service, please check the map to see if the CCA you wish to submit information for, is already on the map. If so, you can give additional information in the comments. In case you wish to dispute any information or need any assistance in filling out the questionnaire, please reach out to [email protected].
